Submit from the Forms Module
- Go to Forms
- Select Create Incident Report Form.
- Use the search bar to find and select the relevant participant.
- Complete the form with accurate details.
- Click Progress Report to Review to submit.
Completing the Report
Before submitting, make sure you have included all relevant and required information. This ensures the incident is recorded accurately and can be reviewed without delay.
Key fields to complete include:
- Date and time of the incident
- Location of the incident
- Description of what occurred
- Type of incident (e.g. behaviour, injury, medication error)
- Witness details or statements, if relevant
- Any supporting attachments such as photos or documents (if needed)
Tip:
Complete your report while details are still fresh and make sure its completed by the end of your shift.
