The purpose of this policy and procedure is to ensure participant exits and transitions from Empowering Connection services are planned, respectful, safe and well-coordinated.

This policy and procedure applies to all staff involved in participant exits, transitions or transfers of supports and meets relevant legislation, regulations and Standards.
