The purpose of this policy and procedure is to ensure participants receiving Community Nursing (CN) services from Empowering Connection receive appropriate, timely and person-centred nursing supports to meet their individual needs and goals.

Community Nursing services work with participants and their families to maintain health, manage existing conditions, promote independence and support carers.

This policy and procedure applies to all staff delivering Community Nursing services and meets relevant legislation, regulations and Standards.

Scope of Community Nursing Services

Community Nursing services may include, but are not limited to:

• care following hospital discharge
• general nursing care
• management of chronic health conditions
• palliative and end-of-life nursing support
• wound care treatment and management
• diabetes management and
• continence management

Each participant accesses supports that are appropriate to their needs, are delivered safely, and support their health, wellbeing and independence.
